Health and Permission Forms

Campers who are not currently enrolled at Villa Duchesne for the 2025-2026 School year must fill out a Health and Permission form and submit electronically to summer@villa1929.org no later than two weeks before the camper's first day of camp.

Failure to submit Health and Permission forms will forfeit the camper's seat at Passport to Summer and a refund will not be issued.
